You must attend one of the mandatory information meetings in order to use the services of the LSU Premedical/Predental Review Committee. Committee Registration cards will only be given out at the informational meetings. One "last chance" informational meeting will be held on Thursday, February 11, 2010 at 5 pm in 103 Williams Hall. This meeting will be for students who were unable to attend one of the meetings held in November. If you attended a meeting in November, you do not need to come to this meeting. Bring your LSU Student ID to the informational meeting in order to record your attendance.

This form is to be completed by LSU faculty/researchers only. Outside evaluators should submit a letter written on their personal/professional letterhead.

For students who plan to enroll in Medical/Dental school in August 2011:

Important Deadline Dates
May 3, 2010 Submission of Registration Card. Must be received in 351 Hatcher Hall by 4:30 pm
May 17, 2010 Early Decision Application Deadline. All committee materials must be turned in to 351 Hatcher Hall by 4:30 pm to guarantee materials are submitted to medical schools by their deadlines
May 27, 2010 Last chance to take MCAT to qualify to use the LSU Committee
June 1, 2010 All committee material is due in 351 Hatcher Hall by 4:30 pm. DAT Score reports must be received by this date.

NO EXCEPTIONS ARE MADE TO THESE DEADLINE DATES FOR ANY REASON.
